Personal Helpers and Mentors (PHaMs)
Break Thru’s community-managed Personal Helpers and Mentors (PHaMs) provides support for people who have difficulties coping with daily activities and connecting with their community. Recovery Mentors who work within the program, share their experiences and provide a strength-based recovery approach that acknowledges everyone can live satisfying and contributing lives.
Break Thru provides support to meet individual choice and needs within their own preferred community setting, whether that is their home or another community environment. The Recovery Mentor assists clients in their ability to manage every day tasks, such as:
- Improving relationships
- Accessing clinical support services
- Linking in with community activities and services
- Developing and improving personal skills and confidence and;
- Setting and achieving life goals.
