Break Thru’s Mental Health Community Support Service (MHCSS)
Mental Health Community Support Services (MHCSS) aims to assist people with a mental health issue to live independently, maintain the best possible social and emotional wellbeing, meet their personal recovery goals and live satisfying lives in the community.
Planned support is available for a negotiated period of time through a key worker.
Support is tailored to meet individual choices and needs and is provided within the individual’s preferred setting, whether that be in their own home or a community environment.
Break Thru works in partnership with carers and families (with the person’s consent), as well as other health professionals, to develop a recovery plan tailored to meet specific preferences.
The recovery plan will provide a number of supports aimed at improving:
- Daily living skills
- Capacity for mental health self-care and self-management
- Relationship and parenting skills
- Physical health
- Social connectedness
- Housing, education and employment (as required)
Where appropriate Break Thru can also provide support for families and carers, including children in a caring role.
