Life Choices Program assists people aged 25 – 54 with moderate to very high support needs to achieve their goals and participate as valued and active members of their community.
The program participants have opportunity to enhance their independence, benefit from ongoing learning and personal development and have support to develop and sustain social interactions and participate in the community.
The Life Choices Program can also offer families and carers a break from their caring role.
The Self Managed Model is one of the ways a person with a disability may choose to use their funding for their day program. In NSW those currently receiving Active Aging, Life Choices or Community Participation may choose to access the Self Managed Model.
Break Thru will work in partnership with people with disability, their family/carers to develop a person centred plan that focuses on individual goals abilities and dreams for the future. The individual will determine the way their money is spent, the activities, experiences they have, how they spend their time and who will support them.
